Adventist Health chooses Emcara Health to meet the needs of Medi-Cal’s most vulnerable members

FRANKLIN, TN.–(BUSINESS WIRE)–Emcara Health, PopHealthCare’s national value-based medical group that provides advanced primary care in nursing homes and vulnerable populations, today announced it is partnering with California-based Adventist Health to expand care in high-risk communities throughout the state. As part of CalAIM’s broader Improved Care Management initiative, the partnership aims to close gaps in care for patients with the most complex care needs by addressing social barriers, such as homelessness, that impact patient health.

Emcara Health will deploy on-site care teams comprised primarily of community health workers (CHWs) to meet patients on their own terms, whether that’s at home, in a homeless camp, or in the emergency room. Once engaged, CHWs conduct a social determinants of health assessment to understand social barriers that may affect a patient’s health, such as food access, stress, transportation, and more. As part of the agreement, Emcara Health nurses and social workers will connect and coordinate preventive care and clinical treatment through Adventist Health. Emcara Health will provide comprehensive support, including transition of care, providing assistance to members and families, and coordinating and referring patients to community and social support services.
