Health Department Conducts Inspections – Sent-trib

The Wood County Health Department focused on Perrysburg, Rossford and Bowling Green for its inspections in late September and the first week of this month.

The next inspections were carried out on September 29.

Little Caesars pizza154 W. South Boundary St., Perrysburg, had two critical and four noncritical violations.

Critical are written procedures and plans that are not maintained and implemented (repetition); and temperature-controlled foods are not kept cold at the correct temperature. Both were corrected during the inspection.

Noncritical are working food containers that are not properly labeled; food industry workers wearing jewelry on hands or arms during food preparation; food worker(s) not wearing a hair restraint (repeat); and non-food contact equipment surfaces are unclean (repeat).

Domino pizza154 E. South Boundary St., Perrysburg, had two critical and five non-critical violations.

Cases where employees are not informed in a verifiable way about their responsibility to report information about their health are critical; and work containers of poisonous or toxic materials that are not properly labeled (corrected during inspection).

Non-critical when the facility does not have an employee with a food safety manager certificate; external opening is not protected; food worker(s) not wearing a hair restraint; stationary equipment not properly sealed or positioned for cleaning; and the food retail establishment does not meet the requirements for level one certification.

City Tap and Attic110 N. Main St., Bowling Green, had two critical and six non-critical violations.

Critical were the inaccessible sink for washing hands; and chlorine sanitizing solution at incorrect temperature and/or concentration (repeat).

Non-critical when the facility does not have an employee with a food safety manager certificate (repeat); utensils and/or equipment not properly rinsed; no disinfectant test kit available; equipment surfaces that do not come into contact with food are unclean; the facility is not kept clean (repeating); and physical facilities not maintained in good condition (repeat).

Holiday pub27355 Carronade Drive, Perrysburg, had two critical and six non-critical violations.

Critical were raw shell eggs not kept at 45° F or below; and temperature-controlled foods are not kept cold at the correct temperature. Both were corrected during the inspection.

Improper food storage was not critical; non-food contact surfaces are not easily cleaned; non-food contact surfaces of equipment are contaminated (repeat); cooking equipment or pan surfaces are unclean; physical facilities are not maintained in good condition; and the food service operation did not have a shift supervisor with a food safety supervisor certificate (corrected during inspection).

Perrysburg Health and Rehabilitation Center, 28546 Starbright Blvd., had two non-critical violations that were unclean non-food contact equipment surfaces (repeat); and the food service operation did not have a shift supervisor with a food safety supervisor certificate.

When checking on a complaint, district Tacos42 LLC, 10584 Fremont Pike Suite 4, Perrysburg, has one critical violation, which is that temperature-controlled foods were not hot, held at the correct temperature (repeat).

A person reported on September 28 that two out of three people became ill immediately after eating/drinking at the restaurant. The inspector spoke to the responsible person and was unable to verify the complaint.

The next inspections were carried out on September 30.

Bob Evans Restaurants LLC1313 Levis Commons Blvd., Perrysburg, had four critical and six noncritical violations.

Cases where employees are not informed in a verifiable way about their responsibility to report information about their health are critical; equipment surfaces that come into contact with food or utensils are unclean; raw shell eggs not held at 45°F or below (corrected during inspection); and work containers of poisonous or toxic materials that are not properly labeled (corrected during inspection).

Non-critical were improperly stored utensils in use (corrected during inspection); equipment components are not intact, tight or properly adjusted (repeat); and physical facilities are not maintained in good condition.

On further inspection, of Pisanello pizza203 N. Main St., Bowling Green, had one noncritical violation, which was physical facilities not maintained in good condition.

On further inspection, Uptown-Downtown162 N. Main St., Bowling Green, had one critical and six non-critical violations.

Critical were the mosquitoes and flies observed in the facility

Non-critical equipment components are not intact, tight or properly adjusted; surfaces that do not come into contact with food are not easily cleaned; surfaces that do not come into contact with food are not cleaned with the necessary frequency; improperly maintained or repaired plumbing system; physical facilities are not maintained in good condition; and the facility is not kept clean.

Carranor Hunt & Polo Club502 E. Second St., Perrysburg, had two critical and two non-critical violations.

Temperature-controlled foods that were not kept hot at the correct temperature were critical; and temperature-controlled foods are not kept cold at the correct temperature.

Unclean equipment surfaces that do not come into contact with food are not critical; and cooking equipment or pan surfaces are unclean.

The next inspections were carried out on Oct. 3.

Pilot Travel Center, 3484 Libbey Road, Perrysburg, has three non-critical non-food contact and cleaning violations as required; improperly maintained or repaired plumbing system; and physical facilities not maintained in good condition (repeat).

On further inspection, Fat Fish Blue Home of fun Bone6140 Levis Commons Blvd., Perrysburg, had one critical and five non-critical violations.

Critically, employees are not informed in a verifiable way about their responsibility to report information about their health.

Non-critical were inadequate equipment available for cooling, heating or storing food; equipment surfaces that do not come into contact with food are unclean; physical facilities are not maintained in good condition; without protective shielding of the lights; and the food service operation did not have a shift supervisor with a food safety supervisor certificate.

Saint Clara Commons12469 Five Point Road, Perrysburg, had five critical and six noncritical violations.

Critical were the chlorine sanitizing solution at the wrong temperature and/or concentration; equipment surfaces that come into contact with food or utensils are unclean (repeat); improper cooling of temperature-controlled food (corrected on inspection); temperature-controlled foods that are not cold, held at the correct temperature (repetition corrected during inspection); and chilled, ready-to-eat, temperature-controlled foods that are not properly dated (corrected during inspection).

Non-critical were inadequate equipment available for cooling, heating or storing food; improper method of cooling temperature-controlled foods; Improper storage of food products; equipment and utensils not air-dried (corrected on inspection); non-food contact surfaces are not easily cleaned; and non-food contact equipment surfaces are unclean (repeat).

Hissho Sushi @ Meijer, 10055 Olde US 20, Rossford, has one critical violation of ready-to-eat, temperature-controlled food not properly disposed of when required. Corrected on review.

Total sport Grill, 10020 S Compass Drive, Rossford, had two non-critical violations, which were that the facility did not have an employee with a food safety manager certificate; and floors, walls and/or ceilings are not smooth and easy to clean (repeat).

Teahouse TM LLC3565 Libbey Road, Perrysburg, had three critical and three noncritical violations.

Critical are cases where employees are not informed in a verifiable way about their responsibility to report information about their health (repetition); food is not properly protected from contamination by separation, packaging and segregation (corrected during inspection); and improper cooling of temperature-controlled food (corrected during inspection).

Non-critical were the lack of towels or a drying device at the hand-washing sink(s) (corrected during the inspection); temperature-controlled foods not defrosted properly (repeat, correct during inspection); and improper food storage.
