Oak Street Health has a health clinic in Glendale Characteristic

Oak Street Health is a fast-growing healthcare option in Arizona and has brought its services to Glendale.

The Glenfair Center is Oak Street’s fourth clinic in Arizona and will provide first aid to patients at Medicare and Medicare Advantage.

“Our model is 100% focused on providing the highest quality primary care we can to Medicare patients,” said Dr. Patrick Wink, Oak Street Health’s Regional Medical Director. “We enter communities that are usually underserved in terms of access to primary care, and we are really excited to join (this) community.”

Oak Street Health provides personalized and preventative care through a model designed to meet the unique needs of older people.

The integrated model includes behavioral health care and support for social determinants and is easily accessible through a combination of center, home and telehealth meetings, as well as a full-time patient support line.

Originally founded in 2012, Oak Street Health jumped into the Arizona healthcare market in January after opening its Apache Junction clinic. The Glenfair Clinic will join the East Mesa and Desert Palms Clinics, which opened in March and April, respectively.

It is expected to open several more locations in Phoenix in Tucson before the end of the year.

“We’re looking at communities that have less access to primary care providers and need people with Medicare,” Wink said. “We strive to enter these communities and serve these patients.”

The company reduced hospital admissions by approximately 51% compared to the Medicare criteria and resulted in a 42% reduction in the 30-day readmission rate with a 51% reduction in emergency room visits, while maintaining a net score for a promoter of 90 for patients, according to a press release.

Wink has been with Oak Street Health since December 2021, and caring for inadequate patients and patients with complex medical problems has always been his passion.

“I feel very strongly that we need high-quality primary care throughout Arizona, and I truly believe that we can not only provide that care with Oak Street, but we can also do it on a large scale to help many, many lives.” , Wink said.

Oak Street Health hires staff from the neighborhoods where the centers operate and invests resources to make a positive impact in the communities it serves. Wink said it was a great place to practice medicine.

This 8,500-square-foot clinic will be added to the list of more than 130 centers in 20 states. The time that Oak Street Health can offer its patients, Wink said, is the most important part.
